Explosion at a nuclear power plant near Kiev, Ukraine, on 26 April 1986. Meltdown of the core was narrowly avoided and a concrete cap used to seal the damaged reactor, but serious contamination of the surrounding area occurred and radioactive fallout was spread across Europe. The accident was caused by human error.
